CASE MANAGEMENT
Frannie Peabody Center provides professional, client-centered, primary case management services to people living with HIV/AIDS in Cumberland and York counties.  Our trained case managers provide referrals, transportation, coordination of care, support and advocacy helping clients meet their medical, psychological, legal and physical needs.
  • In 2008, our case managers spent more than 4,500 hours in direct contact with clients, an increase of 11% from 2006
  • Case managers assess clients at intake and annually thereafter to determine client needs. 
  • Each quarter, clients establish a care plan with their case managers, based on needs identified in the assessment
  • Case managers help clients reach their quarterly goals through referrals, advocacy, support, and coordination of care
  • 72% of all referrals made in 2008 resulted in successful linkage to care or services
  • All clients who did not have medical care and/or insurance coverage at the end of 2007, received both by the end of 2008

Sinclair’s Story
Sinclair found out he was HIV positive in 2004, just after the birth of his son.  After being discharged from the hospital he went to Family Practice on the East side of Portland, where they referred him to Frannie Peabody Center.  As soon as he met his case worker, Kristin Espey, he felt immediately at ease.  Says Sinclair of Kristin, “Kristin is easy to talk to and knows what the client needs before they even have to ask.”  Kristin helped Sinclair navigate the complexities of finding the necessary services in order to live a full life.
