Case Management
Frannie Peabody Center provides professional, client-centered, primary medical case management services to people living with HIV/AIDS in Cumberland and York counties. Our trained case managers provide referrals, transportation, coordination of care, support and advocacy helping clients meet their medical, psychological, legal and physical needs. In spite of a tenuous economic climate, our case management program was able to continue all of our successful supportive service projects such as our women’s group and client newsletter.
- Our case managers spent more than 8,200 hours in direct contact with clients, an increase of 30% from 2009
- Case managers assess clients at intake and annually thereafter to determine client needs
- Each quarter, clients establish a care plan with their case managers, based on needs identified in the assessment
- Case managers help clients reach their quarterly goals through referrals, advocacy, support, and coordination of care
- 99% of clients all clients were enrolled in an insurance coverage program by the end of 2010
