Client Services

People living with HIV in Cumberland or York counties can establish a consistent and supportive relationship with a primary case manager by setting up an appointment for an intake assessment.

During the intake process, clients are asked to identify their needs and set goals.

Case managers offer support, transportation and coordination of care. They help clients prioritize and work toward achieving their goals and link clients to Peabody Center services as well as medical care, housing, and other community resources and information.

Services are delivered at our offices, clients’ homes, at Portland’s Positive Health Care clinic, hospitals, nursing homes, correctional facilities, and in appropriate community settings, based on client need.

The following services are available: to our clients through their case managers:

  • One-on-one support
  • Emergency assistance (when all other resources have been exhausted) for dental care, eye care, and other medical expenses. Income guidelines and an annual cap apply.
  • Emergency assistance with rent, mortgage, and utility payments. Income guidelines and a cap apply.
  • Rental subsidies. Income guidelines and rent standards apply; wait list time varies.
